Thank you so much for your interest in this exciting new role of Head of Clinical Community Services at Ashgate Hospice.

We have been providing specialist palliative and end of life care for patients and their families across North Derbyshire for over 30 years. Last year, we looked after 1,647 patients at home, 363 patients on our inpatient unit, and we supported 585 bereaved family members. I am hugely proud to be the Director of Quality and Patient Care within such an exceptional organisation.

As a member of the senior nursing team, you will work in close cooperation and collaboration with senior nursing and medical colleagues to deliver seamless and high quality care for patients and families, supporting integration and flexible use of resources across Ashgate Hospice.

We have now created this exciting new role of Head of Clinical Community Services to take an important leadership position in our outstanding clinical team. We are looking for someone with a successful track record of:

  • strong clinical and professional nursing or AHP leadership and expertise
  • leading the provision and development of innovative and evidence-based clinical care
  • expert and collaborative practice, providing advice, education and support for other members of a healthcare team as well as patients and families.
  • empowering nursing and therapy staff to provide best practice for patients
  • exploring and implementing new service models to ensure that care remains fit for the future
  • working as part of a clinical and medical leadership team and with a wide range of health and social care professionals and external partners
  • building close, effective and sustainable working relationships with a range of professionals from other provider and commissioning services.

You will be joining Ashgate’s skilled and supportive clinical leadership team and will bring well-developed leadership and interpersonal skills, with the demonstrable ability to inspire staff and volunteers to live our values and to achieve our purpose.
