This is a service with a difference. The Mental Health Community Rehab Team works in partnership with Nottinghamshire Healthcare Trust, providing individuals with person-centred support within the community tailored to their needs. Our Support Workers play an active part in people’s recovery, enabling and sustaining independent living while improving their quality of life, no matter how small the impact may be.

We have an exciting opportunity to be part of the Mental Health Community Rehab Team as a Mental Health Support Worker. You will be an integral part of a dynamic multidisciplinary team offering support to individuals to make choices and live as independently as possible.

The service is for working age adults with mental health needs leaving hospital to enter back into the community. Support will be delivered in their home and in the community and may look different according to the individual you support. One day you might be supporting individuals with their daily living skills, like cooking a nutritious meal in their home. Another day, you could be supporting someone to build confidence in the community, such as through navigating public transport, joining a group activity or other social integration opportunities. This is a partnership with Nottinghamshire Healthcare Trust and you will work alongside Social Workers, Occupational Therapists and Community Psychiatric Nurses’, contributing to assessments and interventions and implementing care plans with the aim of preventing readmission to hospital. 

As this service is based in the community, you will need a valid driving licence and access to a vehicle. All your work mileage will be paid. You will have the autonomy to shape your own workload, so being comfortable with lone working is a must, as well as being computer literate and happy to use technology where required. Above all, you will embody NCHA’s CLEAR values and be willing to make a difference to people’s lives.

 

Shift Pattern

Monday – Thursday 9am-4:30 pm and Friday 9am-4pm. You will not be expected to work weekends or bank holidays however some flexibility may be required based on the needs of the service.

This role is community based predominately in the City and south of Nottinghamshire so a driving licence is essential.
