You must be self-motivated, have a flexible approach to working arrangements and understand working in the community in an individual’s own home. You will receive your rota in advance, and will include working Saturdays, Sundays, and Bank Holidays. Enhancements are paid for Sundays and Bank Holiday working. You must have a genuine interest in promoting independence and supporting an individual to regain their confidence and skills.
This may be after a period of rehabilitation in a residential setting or on returning home from hospital. We also receive community referrals for customers who may be struggling to manage at home.
Applicants must have a clean Full UK Driving Licence and access to own vehicle with business insurance as the post requires travelling within Middlesbrough a mileage allowance is payable for this.
The main areas of responsibilities of the post include:
To take responsibility for the administration of medication, the necessary recording, and audits.
To undertake risk assessments including the home environment and medication.
Working without direct supervision and responsible for individual progress and reporting back to the lead person on individual’s performance, behaviours, strengths, needs and concerns.
To liaise with other professionals, e.g., GP’s, consultants, district nurses, social workers, care agencies and others.
To deliver formal and informal supervision to Reablement Officers and perform direct observations.
To cover sickness and holidays it may be necessary for you to be ‘on-call’ from home to support the Reablement Officers on shift.
Visiting customers in their own homes, hospital ward or care home to gather and record information.
Undertaking audits and risk assessments as necessary (training will be provided).
Ensuring Reablement Officers have all the information they need to provide support to customers supporting customers with their medication, washing and dressing, food preparation, outdoor and indoor mobility, and daily household chores.
To provide on-call arrangements to cover sickness and holidays.
To observe and record customer progress with identified goals using our electronic support plan, training and support will be provided.
To use technology and systems safely and effectively, training and support will be provided.
To work collaboratively with your colleagues, Occupational Therapists, Case Managers, and other professionals to achieve the best outcome for our customers.
To follow policies and procedures of Middlesbrough Council including being responsible for your own, your colleagues and our customers.
Health and Safety, GDPR and Covid-19 policies and procedures.
Understanding CQC’s requirements.
To undertake mandatory training including Food Hygiene, First Aid and Medication Management and a range of other training.
Attend regular supervision sessions.
