A rare opportunity has arisen for a Social Worker to join our multi- disciplinary team at the Hospice of the Good Shepherd. You will be joining us at a pivotal time as a key member of the multi-disciplinary team dedicated to improving palliative and end of life care for patients and families. Drawing upon your recent social work experience you will have the opportunity to develop social work and safeguarding provision at the Hospice. The successful candidate will have a positive outlook, be personally robust and able to remain resilient when working with sustained distress. You will demonstrate the ability to work efficiently whilst recognising the need for empathy, sensitivity and confidentiality when working with colleagues and the public.

The post holder will:

  • function as part of the hospice multi-disciplinary team, contributing to holistic plans of care, offering provision which supports the complex social, psychological and emotional needs of people who are approaching the end of life or living with a palliative diagnosis.
  • use patient identified goals and an understanding of their social and psychological needs to contribute to the multi professional care and discharge planning for patients.
  • have a key role in initiating discussions and supporting the implementation of Advance Care Planning Decisions.
  • be the designated safeguarding lead supporting the MDT in appropriately applying the guidance of the Mental Capacity Act and Care Act and always working within their frameworks to safeguard the rights and autonomy of patients and providing mandatory safeguarding training
  • understand the impact of grief and loss on patients and their families.

 

and will be able to:

  • demonstrate their ability to identify and attend to the emotional, psychological, and cultural needs of patients, and those important to them
  • demonstrate their ability to work as part of a wider integrated team  with strong leadership skills

 

You will be joining a friendly and welcoming team with a collective aim of positively enhancing the lives of the individuals in our care. You will be supported through a specific induction programme.

The Hospice is a great place to work and volunteer. Based in Backford, Chester (with retail shops in Chester and Ellesmere Port), the Hospice provides specialist palliative and end-of-life care in a welcoming and compassionate environment.

Our specialist team of medical and nursing staff deliver end of life care to our patients and support their families, carers and friends but our services are varied across a number of disciplines and reflect the diverse nature of our care and support. In addition to Doctors, Nurses and Health care roles, we employ staff in a wide range of roles including Fundraising and Retail in our charity shops, Housekeeping and Catering, Social Work and Family Support such as Counselling and Spiritual Care, Living Well / Day Services as well as Business Support functions such as Administration, People & Development and Finance.

We also have a large number of Volunteer roles in all these service areas. We have been supported by our volunteers ever since the hospice was established and we now have over 400 people contributing their time in so many different ways. Every hour volunteered allows us to continue providing specialist care and we are always welcoming applications for new volunteers so please get in touch.
